Where are HEALS clinics located?
Is HEALS the same as a school nurse?
No. HEALS provides full primary health care services, including diagnosis, treatment, and referrals, while school nurses typically provide basic care and support.
Who is eligible for services?
HEALS clinics are located within participating schools and provide primary care services to all individuals in the service area without regard to ability to pay or insurance status. Clinic sites and scheduling are structured around the school day to maximize access for enrolled students and their siblings, and services are available to all community members seeking care.
How do I enroll my child?
Parents or guardians must complete enrollment forms and provide consent before services can begin. Assistance is available if needed.
Why is school-based health care important?
School-based primary care is an evidence-supported delivery model that addresses documented barriers to health care access, including lack of transportation, inability to pay, and caregiver work schedule conflicts. Locating clinical services within school facilities increases utilization rates among pediatric populations who would otherwise experience delayed or foregone care. The HEALS clinic model supports improved health outcomes through timely treatment of acute illness, preventive care including well-child visits and vaccinations, vision screening and corrective lens provision, and coordination with community resources including nutrition support programs. These services collectively reduce illness-related absenteeism, support academic engagement, and strengthen the health infrastructure of the communities in which HEALS operates.
What proof of income should I provide if I do not have a traditional job, am self-employed, or earn income through informal work?
Patients who cannot provide standard proof of income may complete a Self-Declaration of Income Form, which can be used to document their household income for eligibility and Sliding Fee Scale purposes.
